Modification of the Applicant’s/Registrant’s Name/Address
1) Order Letter;
2)Original Power of Attorney (signature or seal for foreign natural person or legal person and Chinese natural person; company seal for Chinese legal person);
3)Copy of Notification on Acceptance for registration application or copy of Registration Certificate;
4)Original or notarized copy of Domestic Certificate for Modification of the Applicant’s/ Registrant’s Name, in which the old and new names should be both indicated (in case only the Chinese name of the foreign applicant is modified, no Certificate for Modification is needed).
Note: When applying for modification of the applicant’s/registrant’s name/address, the applicant/registrant shall conduct such modification in respect of all his applied/ registered marks in P.R.C. at the same time.
